comms: remove 'Created by' lines

Remove # $FreeBSD$ from Makefiles.

Do not abuse INSTALL_MAN when installing documentation, examples, and
other miscellaneous files that are not actually manual pages (part 2).

MASTER_SITES cleanup.

- Replace ${MASTER_SITE_FOO} with FOO.
- Merge MASTER_SITE_SUBDIR into MASTER_SITES when possible. (This means 99.9%
  of the time.)
- Remove occurrences of MASTER_SITE_LOCAL when no subdirectory was present and
  no hint of what it should be was present.
- Fix some logic.
- And generally, make things more simple and easy to understand.

While there, add magic values to the FESTIVAL, GENTOO, GIMP, GNUPG, QT and
SAMBA macros.

Also, replace some EXTRACT_SUFX occurences with USES=tar:*.

Cleanup plist

Rename comms/ patch-xy patches to reflect the files they modify.

Support STAGEDIR.

Add NO_STAGE all over the place in preparation for the staging support (cat:
comms)

. add license (GPLv2);
. bump PORTREVISION;
. NOPORTDOCS -> PORT_OPTIONS:MDOCS;
. mute mkdir command;
. do not surround PORTDOCS with .if operator;
. trim Makefile headers.

- Get Rid MD5 support

For ports maintained by ports@FreeBSD.org, remove names and/or
e-mail addresses from the pkg-descr file that could reasonably
be mistaken for maintainer contact information in order to avoid
confusion on the part of users looking for support. As a pleasant
side effect this also avoids confusion and/or frustration for people
who are no longer maintaining those ports.

Return laszlof's ports to the pool due to lack of time.
