Mark QT4 ports/functionality for removal on 2019-03-15

While here, chase some KDE4 ports and functionality, these are scheduled for
removal on 2018-12-31. Change the default option/flavor to QT5 where applicable
or use alternative toolkits like GTK.

Make the port build with the upcoming Qt 4.8.x series.

Since 4.8.0 [1], Qt itself does not automatically include GLU's
headers and libraries with its OpenGL module, so applications which
relied on Qt to get these need to be adjusted.

PORTREVISION bumped since we now explicitly depend on GLU via
USE_GL=glu.

Sigh, recovert the two patches to DOS line endings.

The files they are patching have DOS line endings, so... Brown paper bag to me.

Convert the patches added in the previous commit to Unix line endings.

Make the port build with gcc 4.6 (and possibly other compilers).

Use QFoo(...) instead of QFoo::QFoo(...), which is a wrong way of calling the
constructor.
